Our comprehensive range of aging-in-place services includes mobile occupational therapy, caregiver training and education, home safety assessments, hospital discharge preparation, and diabetes self-management.  With a highly personalized approach, we strive to optimize our clients' living environments, enhancing their overall quality of life. Prediabetes Course

The Call 

Give us a ring at 704-387-6420 for a complimentary 15-minute one-on-one conversation. This quick chat allows us to understand your unique needs and concerns, enabling us to tailor our services specifically to you. Following this, we'll schedule your initial appointment

The Appointment 

Before the initial appointment, we'll need your Medicare number for benefits verification. Don't worry if you don't have Medicare; contact us to explore available options. The session includes a comprehensive review of your health history, allowing us to set meaningful goals and create a personalized plan just for you! Our Occupational Therapy, Caregiving, and Home Safety Assessments happen in the comfort of your home, while diabetes coaching appointments are conducted virtually

The Plan

If necessary, Welldom will reach out to your physician to confirm and sign off on your personalized plan of care. Together, we'll work towards achieving your goals, ensuring you get the results you desire.

Originally founded in 2019 by a physical and occupational therapist, Welldom provides an alternative to the costly, complex managed care system. Welldom brings a client-centered, compassionate approach to those in the community that we serve.
